Description
This course will combine all the learning objectives from the previous 9 modules. The main aim is to develop an implementation plan, based on the tools and knowledge acquired during the year. Throughout the four weeks, the course will use live lectures, small group work, discussions and innovative engagement tools to demonstrate to students the challenge and difficulties of implementation and change management in practice. It will give important input for year 2 and further practice, and will also cover the topics which may not have been centre stage up until now, including an outcomes framework, workforce and community engagement, as well as leadership and management. Students will be expected to engage with stakeholders and relevant communities, build on their own experience in project case 2 and present their implementation plans to an interested audience in a way that convinces them of the necessary changes suggested. Live online lectures on real life cases will complement the background material available for independent study. As such, it will come as close as possible to a real life situation.
Course objectives
Upon successful completion of this course, the student:
can understand and apply the essentials of implementation theories for a PHM approach (from theory to practice).
can analyze elements of a successful and failed implementation in various regions or populations around the world.
can synthesize all elements of the practical guide and create their own implementation plan.
can create their own outcomes framework for the implementation process.
The final aim of the course is for students to experience the challenges of co-designing a realistic PHM implementation plan together with key stakeholders and communities, and convincing the payers to invest in it.

Assessment method
Students are assessed according to the following three obligatory components:
Week 1-3 – Online: Completion of weekly assignments (20%, no required minimum grade)
Week 4 – On Campus: Group presentation (30%, no required minimum grade)
Final assignment (50%, required minimum grade: 6,0).
All components combined make up the grade for the course. It is compulsory to participate in each of the components in order to receive a grade.

A final grade of 6,0 minimum is considered sufficient. If the result is less than 6,0 or if the student did not participate in one of the components, the student is given the opportunity to retake the assessment as one assignment that covers all the learning goals of the course.
Final grades between 5,0 and 6,0 will be rounded:
5,0-5,4 → 5,0
5,5-6,0 → 6,0
Reading list
The reading list can be found on Brightspace. The material consists of presentations and pdf files. There is no need to purchase literature, as the presented material is not commercialized.
