Panel Management Next Level

Vak
2024-2025

Admission requirements

Successful completion of Fundamentals of Population Health Management and Study Design is advised.

If you are not enrolled in the MSc programme Population Health Management but you consider taking this course as an elective, please contact our study advisor.

Description

How can you, as a healthcare professional, identify patients that share the same risk of an adverse event? How do you transform your healthcare practice in such a way that you are not only able to provide proactive care but you are also able to improve the health of populations at risk? In this course you will learn how to apply the panel management approach to tackle these questions. You will learn how to empanel (patient)groups that share the same risk (Module 1) and how you can allocate the appropriate intervention to each panel of patients (Module 2). After this, you will dive into ways to transform current workflows for optimal in-reach and out-reach care, both at the cultural and behavioral change level (Module 3). As a final step you will learn how to evaluate both effectiveness and process based on the triple aim (Module 4). This will give you insight into what can be improved in the next panel management cycle.

This course will help you gain an understanding of the panel management approach and enables you to change from reactive to proactive care. In the online weeks, the four steps of the panel management process are covered in four different modules. Throughout the modules you will design your own panel management program. In the on-campus week, you will apply your knowledge to a case presented by one of the stakeholders. And finally, in the individual week, you will write a grant application based on the panel management program you have designed in the online weeks.

Course objectives

Upon successful completion of this course, the student:

  • can build an algorithem to identify people sharing the same risk of an adverse event in healthcare in the population and allocate those to an administrative subgroup. (empanelment)

  • can choose and design the appropriate intervention to each panel of patient who share the same risk, aiming at reducing the risk of an adverse event in healthcare (choosing the appropriate intervention)

  • can identify those who missed the appropriate intervention at the chosen time. (Surveillance of caregaps)

  • can design an effect and process evaluation in terms of risk reduction of meeting defined adverse events (evaluation)

All course and group schedules are published on MyTimeTable.

The exam dates have been determined by the Education Board and are published in MyTimeTable.
It will be announced in MyTimeTable and/or Brightspace when and how the post-exam feedback will be organized.

Assessment method

Students are assessed according to the following three obligatory components:

Part 1 (week 1-2 – Online): Peer review assessment (20%, no required minimum result)
Part 2 (week 3 – On Campus): Group presentation (30%, no required minimum result)
Part 3 (week 4 – Final week): Final assignment (50%, required minimum result: 6,0)

Partial grades are rounded to 1 decimal place.

All components combined make up the final grade for the course (with 1 decimal place, except for grades between 5,0 and 6,0). If the result of part 3 is below 6.0, the final grade will be capped at a 5.0. It is compulsory to participate in each of the components in order to receive a grade.
Details on the assessment can be found in the assessment plan on Brightspace.

A final grade of at least 6,0 is required to pass the course. If the final grade is less than 6,0 or if the student did not participate in one of the components, the student is given the opportunity to retake the assessment as one assignment that covers all the learning goals of the course.

Final grades between 5,0 and 6,0 will be rounded:

  • 5,0-5,4 → 5,0

  • 5,5-6,0 → 6,0
