Description
This course will combine all the learning objectives from the previous 9 modules. The main aim is to develop an implementation plan, based on the tools and knowledge acquired during the year. Throughout the four weeks, the course will use live lectures, small group work, discussions and innovative engagement tools to demonstrate to students the challenge and difficulties of implementation and change management in practice. It will give important input for year 2 and further practice and will also cover the topics which were not yet discussed in other modules, including how to use a Theory of Change and a Logic Model to devellop an implementation plan, workforce and community engagement, as well as leadership and change management theory and practice. Students will be expected to build on their own experience in project case 2 and present their implementation plans during a PHM symposium, which is open to an interested audience and for which the students design the programma and format of to engage with the audience. Live online lectures on real life cases will complement the background material available for independent study. As such, it will come as close as possible to a real life situation.
Course objectives
Upon successful completion of this course, the student:
Recognizes different implementation strategies in PHM cases
Analyses challenges and barriers for implementing a real life PHM case in the health system
Compares and contrasts how several PHM initiatives can be implemented at the same time
Uses a theory of change for a real life case as a basis for their engagement strategy
Creates a logic model for a real life case to plan the implementation process
Demonstrates critical appraisal skills
Demonstrates the competence necessary to work in population health management
The final aim of the course is for students to experience the challenges of co-designing a realistic PHM implementation plan together with key stakeholders and communities and convincing the payers to invest in it.

Assessment method
Students are assessed according to the following four obligatory components:
Part 1 (week 1-4 – Online and On Campus): Completion of weekly assignments (no required minimum result)
Part 2 (week 3 – On Campus): Group presentation self-assessment (30%, no required minimum result)
Part 3: Final group assignment (50%, required minimum result: 6,0)
Part 4: Final individual assignment (20%, required minimum result: 6,0)
Partial grades are rounded to 1 decimal place.
All components combined make up the final grade for the course (with 1 decimal place, except for grades between 5,0 and 6,0). If the result of part 3 is below 6.0, the final grade will be capped at a 5.0. It is compulsory to participate in each of the components in order to receive a grade.

A final grade of at least 6,0 is required to pass the course. If the final grade is less than 6,0 or if the student did not participate in one of the components, the student is given the opportunity to retake the assessment as one assignment that covers all the learning goals of the course.
Final grades between 5,0 and 6,0 will be rounded:
5,0-5,4 → 5,0
5,5-6,0 → 6,0
Reading list
The reading list can be found on Brightspace. The material consists of presentations and pdf files. There is no need to purchase literature, as the presented material is not commercialized.
